Return Policy

At Mays Dermatology & Cosmetic Center we pride ourselves in providing excellence in quality of care. This includes our medical grade products and cosmetic services. In order to control the quality of our products and provide you with timely services on your scheduled procedure, all cosmetic sales are FINAL.  Our products are prescription strength and follow the same regulation as pharmacy purchases. Cosmetic procedures require full payment prior to booking due to inventory re-stocking, blocking of staff time and disposable parts/ equipment.

This is also stated clearly on our signed financial policy. If you purchase a defective item however, we are happy to replace it for you free of charge.

Telehealth Services

Now you can receive care from the comfort of your home, office, or on the go. We understand that life gets busy, so we offer telephone visits when your unable to make it into to the office. This service is only available for established patients ONLY. Up to 3 visits per year. Please click the link below to start the process and a staff member will reach out to you with further instructions.

Cancel/Reschedule Appointment

Please note: Cancel/reschedule requests MUST be placed online or via speaking to one of our staff members in person no later than 24 hours prior to appointment to avoid non refundable fee. Monday appointments must be cancelled no later than that proceeding Friday same time. This option only applies to MEDICAL appointments ONLY. To alter cosmetic or surgery appointments please call us directly at 502-384-6544.

Referral Form for Providers

REFERRING PROVIDERS: Thank you for referring your patient to us. Please fax the completed referral form below. All information on the form is required. Our staff will process the referral and contact the patient directly to schedule their appointment. Alternatively, patients can access our easy online scheduling system by visiting the website and clicking on the BOOK APPOINTMENT link.
