Insurance & Billing

We participate with most insurance plans. All patients are encouraged to check eligibility and benefits with their insurance carrier prior to appointment. We do not guarantee coverage or in-network status. If you do not have health insurance, we are still happy to see you. We offer a simple self-pay fee schedule available upon request.

* Please note: based on your insurance plan and deductible, payment for your entire visit and/or in-office procedures may be required at time of service. Photo ID and valid insurance card required at every visit.

Cosmetic & Cash pay Patients: Please be aware: We do not accept checks as method of payment. All payment is due at time of service or scheduling. (For services requiring disposable purchase/ blocked staff time full payment is required at scheduling). All cosmetic sales are final. 

NO SHOW/CANCELLATION POLICY: We love providing the luxury of same day appointments with a Board Certified Dermatologist. Some offices require credit card on file or deposit to book your appointment. We do NOT do this. However, in order to protect the dedicated appointment slot, we enforce a strict cancellation/no show policy. Cancellations less than 24 hours (weekdays) for ANY reason will result in a fee. This fee is non-refundable and covers the staff in preparation of your appointment and the time slot reserved for you. This policy is also noted on your required signed paperwork. We appreciate your understanding in this matter.

Cancel/Reschedule Appointment

Please note: Cancel/reschedule requests MUST be placed online or via speaking to one of our staff members in person no later than 24 hours prior to appointment to avoid non refundable fee. Monday appointments must be cancelled no later than that proceeding Friday same time. This option only applies to MEDICAL appointments ONLY. To alter cosmetic or surgery appointments please call us directly at 502-384-6544.

Referral Form for Providers

REFERRING PROVIDERS: Thank you for referring your patient to us. Please fax the completed referral form below. All information on the form is required. Our staff will process the referral and contact the patient directly to schedule their appointment.
